NYSED Sample Impartial Hearing Request Form
This sample form may be
used to submit a complaint (also known as a request for an impartial
hearing) to resolve a disagreement about the referral, evaluation or
placement of a student or regarding the provision of a free appropriate
public education for a student under the Individuals with Disabilities
Education Act.

You may also wish to contact the
Community Mediation Center (CMC). They provide alternative dispute
resolution services to residents of Suffolk County. CMC uses trained mediators
and arbitrators to handle a wide range of disputes, with the goal of reaching
legally binding agreements.

IDEA 2004 or the Individuals with
Disabilities Education Act has recently been re-authorized by the
federal government. Below are links to NYSED Commissioner's
Regulations which define how New York State will carry out the law (IDEA 2004)
in regards to educating students with disabilities:
